Discharge to assess (D2A)

The Discharge to Assess Service is for individuals who are resident in Stockport or registered with a Stockport GP surgery.

The Team:

  • Supports the safe transfer from hospital under the ‘Home First’ ethos where appropriate.
  • Maintain care to ensure that an individual is safe and able to meet needs with community provision.
  • Assess in own home within 4 hours of discharge.
  • Complete assessments, provide care input and initiate onward referrals as appropriate.

Patients are assessed at home by a combination of Trusted Assessor, Nursing, Physiotherapy or Occupational Therapy staff.

No. It’s a step-down from hospital. The team makes contact within 4 hours of arriving home and completes a full care assessment.

No, call frequency is determined by the D2A team after the full assessment.
