The Community Advanced Clinical Practitioner (ACP) team provide a holistic, home-based service for patients with complex health needs who are at risk of hospital admission or who have recently experienced hospital admissions. They work with each individual and their carers/families to understand health and wellbeing needs and explore options for treatment and support.
The aim is to improve health and well-being and prevent avoidable hospital admissions by planning for the future and promoting the right care taking place at the right time. Patients will be offered an Enhanced Case Management (ECM) plan if their health is worsening in order to safely prevent unnecessary hospital admissions.
Community service
All patients who are referred to the ACP team must be 18 years and over and registered with a Stockport GP.
They must have two or more unstable long-term conditions (which can include frailty) and/or two or more hospital attendances/unplanned admissions in the previous month.
The ACP team are based in the community and carry out assessments and reviews in the patient’s home, including care homes.
