The Team

Provide a short-term rehabilitation service to adults with a physical disability or long-term condition.

  • Assess and treat individuals within their own home, including residential and nursing homes.
  • Set realistic and achievable goals with the individual, with regular discussions and reviews of said goals.
  • Provide indoor mobility aids, home exercise programmes, equipment and initiate onward referrals as appropriate.

About our service

Patients are assessed in their home by a Physiotherapist or an Occupational Therapist, depending on their specific needs and reason for referral.

No, the Adult Community Therapy Team is not an urgent response service. All referrals are reviewed in terms of individual needs and urgency, and an appointment is made within a reasonable timeframe.

No, the Adult Community Therapy Team is a short-term service, with the ability to provide a maximum of eight therapy sessions. The therapist will set an agreed plan with the patient, which will include a home exercise programme for the patient to complete in-between the therapy sessions. The sessions are booked in accordance with the patient’s individual rehab needs, with most appointments being a minimum of one week apart. The service is unable to provide daily therapy input.
