About the service
Most services offered by Diabetes and Endocrinology are offered by way of outpatient appointment, although some inpatient services are provided for patients with complications.
Within the diabetes service there is a dedicated team of specialist nurses, a specialist midwife, specialist dieticians, high risk podiatrists and a team of diabetes educators that work across the acute and community settings. The whole team provide training and support for staff and for patients with Type 1 and Type 2 diabetes.
Our expertise
Stockport NHS serves a population of 304,000. The prevalence of diabetes runs just below the national average at 7.23% and in 2012, 16,850 people in Stockport had a diagnosis of diabetes (age 16+). The majority of these patients are cared for and managed within primary care but those with complex needs may be referred to the diabetes specialist nursing service.
The community diabetes specialist nursing service comprises of nurses who concentrate their skills and experience offering education, support and advice to patients with both Type 1 and Type 2 diabetes. The nursing service compliments the consultant service based within the hospital and together they offer holistic assessment and management.
There are six core functions delivered by the service:
- Pregnancy – Type 1/Type 2 and Gestational diabetes
- Young persons service including transition from paediatrics
- Insulin pump initiation and management
- Alternative injectable therapies for complex patients
- Insulin therapy and management for complex patients
- Patient/staff education
X-Pert Diabetes Programme
The X-PERT Diabetes Programmes is an award winning six-week group education programme for Type 2 diabetes. It will help you understand more about diabetes and how you can manage it through lifestyle changes. It is suitable for newly diagnosed and those who have had diabetes for a while but want to learn more.
Each programme is run by 2 experienced health professionals for a 2 ½ hour session each week for six weeks. The X-Pert is run on different days in various venues around Stockport.
Topics covered each week are as follows:
- Week 1 – What is Diabetes?
- Week 2 – Weight Management
- Week 3 – Carbohydrate Awareness
- Week 4 – Understanding Food Labelling
- Week 5 – Possible Complications of Diabetes
- Week 6 – Are you an X-PERT game
You will need to be registered with a Stockport GP to attend a course but you can find out more and refer yourself to this FREE course by contacting us on 0161 835 6689.
As well as X-Pert for Type 2 Diabetes,Stockport also has a bespoke education programme entitled SOCCER – which stands for Stockport Optimising Carbohydrate Counting, Education & Results and is a course for people with Type 1 diabetes.
This course runs over a 4 week period with 4 x 8 hourly sessions. Access to this course is via GP or Practice Nurse referral to the Diabetes Nursing team at Kingsgate House contact 0161 204 466.
