The Community Neuro Rehabilitation Service is a specialist team who provide community-based rehabilitation for adults with neurological conditions.
About our service
The Community Neuro Rehabilitation Service provides multidisciplinary rehabilitation and management for all neurological conditions including brain injury, stroke, multiple sclerosis, motor neuro disease and spinal cord injury.
Our specialist team includes:
- Admin
- Service Lead
- Physiotherapists
- Occupational Therapists
- Speech and Language Therapists
- Clinical Psychologists
- Nurses
- Assistant Practitioners/ Therapy Assistants
- The service provides interventions to address individuals’ own goals and to support their own long-term management of their condition.
This may include:
- Movement analysis and physical rehabilitation including arm, mobility, balance
and walking - Rehabilitation of functional daily activities
- Sensory assessment and rehabilitation
- Cognitive assessment and rehabilitation
- Neuropsychological assessment and rehabilitation
- Communication assessment and rehabilitation
- Swallow assessment and management
- Spasticity management
- Postural management and splinting
- Advice regarding the management of long term conditions for patients and their carers
The team works closely with local neighbourhood teams, other professionals, family and carers to provide holistic care.
Assessed and rehabilitation can take place in a patient’s own homes, clinics settings or other appropriate community facilities to support a patient’s goals.
The team deliver group sessions alongside individual patient rehabilitation sessions.
Referral Criteria:
- Aged 16 or above
- Have a newly diagnosed neurological condition or a change in their needs due to their neurological condition
- Be registered with a Stockport GP
- Requires neuro-rehabilitation in the community setting, which is goal oriented to maximise outcomes
- Be able to engage in and benefit from the rehabilitation services offered
- Be willing to consent to assessment / intervention or, if not able to consent, it has been established that the assessment is in the client’s best interests
- All new Stroke patients to be referred to the service on discharge from hospital
