Intermediate care at home (ICaH)

Intermediate Care at Home (ICaH) is an assessment and rehabilitation team whose focus is promoting and assisting a return to independence, tailored to each individual’s unique needs and goals.

The service is provided for anyone with a Stockport General Practitioner (GP) or a Stockport resident, over the age of 18 and accepts referrals from hospitals, the Discharge to Assess Team and Discharge to Assess bed-based units.

Community service

We are an assessment team focused on promoting independence. We create a goal-oriented plan at your assessment and refer you to community services if ongoing care is needed.

Trained Senior Support Workers who assist with independence and wellbeing.

Due to large caseloads, exact times can’t be given, but visits are scheduled in time windows. Time-critical medication needs are prioritised.

Frequency is based on your needs. All staff incorporate therapeutic techniques into visits so every interaction supports progress.

Active participation in your care helps restore independence faster.

It means you’ve regained independence in certain tasks — a positive sign of recovery.
